Hospice RN Case Manager

As a Hospice RN Case Manager with Interim HealthCare of Oklahoma City, you'll manage your own caseload, build real relationships, and deliver the kind of one-on-one care that hospice is meant to be. Requirements
- Active RN license in Oklahoma
- Hospice experience required
- Knowledge of hospice regulations and the Medicare hospice benefit
- Strong clinical judgment, communication skills, and a heart for end-of-life care
- Reliable transportation and a valid driver's license
Responsibilities
- Manage a hospice caseload, coordinating care for patients and their families
- Complete assessments, develop and update plans of care, and document visits
- Manage symptoms and comfort, and educate families on what to expect
- Work alongside the hospice team, physicians, aides, social workers, and chaplains
- Be a steady, compassionate presence for patients and families at end of life
