About The Position

St. Mary's Home care & Hospice is seeking a full time Hospice RN Case Manager. This role is responsible for case managing a team of hospice patients. The RN case manager will collaborate with an interdisciplinary team and attend weekly Interdisciplinary meetings. The RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. The position is full time primarily Monday-Friday with no on call or weekends. St. Mary’s Home Care and Hospice is in Athens, GA and is a member of Trinity Health at Home, a national home care, palliative care and hospice organization serving communities in twelve states. Requirements

Graduate of an approved nursing education program
Licensure as a Registered Nurse in the State of Georgia
2 years experience as a professional nurse in acute care, home care or ER experience required

Nice To Haves

Home care experience preferred but not required
EPIC experience preferred

Responsibilities
