About The Position

As an RN Case Manager at Vitality Hospice & Palliative Care, you will be the clinical lead for your assigned patients, supported by a strong interdisciplinary team. This role is designed to prevent burnout with a manageable caseload, dedicated support staff, and leadership focused on sustainable workloads. You will conduct comprehensive nursing assessments, develop and manage individualized plans of care, collaborate with various healthcare professionals, educate families, participate in IDT meetings, and maintain timely documentation.

Requirements

• Active New Jersey RN License
• Hospice or home health experience strongly preferred
• Strong clinical judgment & communication skills
• Valid driver’s license & reliable transportation

Responsibilities

• Conduct comprehensive nursing assessments (with Admissions Nurse support)
• Develop and manage individualized plans of care
• Collaborate with physicians, LPNs, social workers, chaplains & aides
• Educate families on symptom management and disease progression
• Participate in IDT meetings
• Maintain timely, compliant documentation
