This opportunity is with a well-established hospice team in the North Suburbs of Chicago that is focused on creating a more sustainable and supportive environment for nurses doing this important work. What makes this role different A team that understands the emotional and clinical demands of hospice care Structured support, including dedicated after-hours nurses (you’re not carrying everything alone) Thoughtful approach to scheduling and patient coordination The ability to work independently while still feeling supported The role As a Hospice RN Case Manager, you’ll manage and coordinate care for patients in a home-based setting, working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life.

Requirements

• Active RN license in Illinois
• 1+ year of hospice or home health experience
• Experience in case management or care coordination
• Strong clinical judgment and ability to work independently
• A passion for patient-centered, end-of-life care
• AHA BLS (required)
• AHA ACLS (required)

Responsibilities

• Managing patient care in hospice settings
• Coordinating with physicians and care teams
• Supporting patients and families through all stages of care
• Providing leadership and guidance within the care team
