About The Position

As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life. This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Requirements

  • Registered nurse expertise.
  • Ability to practice the full scope of nursing.
  • Ability to build deep, meaningful relationships.
  • Ability to build rapport with patients, families, physicians, facilities, and other interdisciplinary team members.
  • Autonomy to coordinate comprehensive care for caseload.
  • Ability to work collaboratively with an interdisciplinary team.
  • Daily travel within the service area (Wayne County / Detroit) to provide care to patients in their homes, assisted living facilities, or nursing homes.
  • Successful completion of a background check.
