Hospice RN Case Manager (Registered Nurse) – Full Time Monday–Friday schedule + rotating on‑call Join a compassionate, patient‑centered hospice team where you can provide meaningful care and support to patients and families. Arc Hospice & Palliative Care is seeking a Hospice RN Case Manager to deliver skilled, personalized nursing care aligned with each patient’s goals and needs.

Requirements

  • Graduate of an accredited nursing program; Associate degree or higher preferred
  • Current RN license in the state of practice or valid multistate compact license
  • Minimum one (1) year of hospice, palliative care, or home health experience preferred
  • CPR certification and valid driver’s license required
  • Strong clinical judgment, documentation accuracy, and communication skills
  • Knowledge of hospice regulations and Medicare Conditions of Participation preferred
  • Ability to work independently while collaborating across interdisciplinary teams
  • Excellent communication, critical thinking, and organizational skills

Responsibilities

  • Complete initial and ongoing assessments of patients’ physical, emotional, psychosocial, and environmental needs
  • Coordinate with physicians for certification and recertification of terminal illness per Medicare regulations
  • Serve as the clinical point person for assigned patients, including symptom escalation, advanced care planning, and advocacy
  • Collaborate with all providers and support systems to ensure seamless, compassionate hospice care
  • Develop and update individualized care plans with patients, families, and the IDG
  • Provide skilled nursing care and document interventions and outcomes accurately and promptly
  • Ensure documentation meets regulatory and billing compliance standards
  • Educate patients and families on disease progression, symptom management, medications, and hospice philosophy
  • Supervise hospice aides and revise aide care plans as needed
  • Participate in IDG meetings with physicians, social workers, chaplains, and bereavement staff
  • Communicate effectively with the interdisciplinary team and clinical leadership
  • Participate in the on‑call rotation and respond to urgent needs
  • Escalate care needs to the medical director or covering provider when appropriate
  • Perform other duties as assigned

  • On-call pay equivalent to 15 hours per day for simply being on call, even if you do not receive any calls or provide patient visits during that time.
  • Additional compensation may apply for hours actively worked while on call, in accordance with company policy.
