About The Position
Inspire Hospice and Palliative Care is seeking a compassionate and dedicated RN Case Manager to join their team. This role involves providing care and support to patients and their families during their end-of-life journey. The RN Case Manager will work with patients in their homes, assisted living facilities, or skilled nursing homes.
Requirements
- Valid GA RN license
- Driver's license
- At least one year of experience in hospice or home health

- Hospice or home health Jedi
- Certified Hospice and Palliative Nurse (CHPN)
Responsibilities
- Be a steady source of comfort and support to patients and families
- Help families understand disease progression, self-care, and nutrition
- Collaborate with the interdisciplinary hospice team to create and follow patient care plans
- Manage symptoms and advocate for patient comfort
- Maintain timely and accurate clinical documentation
