About The Position

JourneyCare Hospice is seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care. They offer a different approach with manageable caseloads, strong interdisciplinary support, and the time needed to care for patients and families. This role offers a consistent Monday-Friday schedule with only one weekend rotation every 12 weeks, focusing on quality of care over visit volume, and supportive leadership. The territory includes Mundelein, Libertyville, and Third Lake, IL.

Requirements

• Graduate of an accredited RN program
• Active Illinois RN license
• Current CPR and First Aid certification
• Valid driver’s license and proof of insurance

Nice To Haves

• Hospice or home health experience preferred
• Strong clinical judgment and communication skills

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Maintain timely, accurate documentation
• Supervise Home Health Aides in compliance with regulations
