About The Position

Concho Hearts Hospice RN Case Manager. Concho Hearts Hospice Mission Statement is to provide compassionate care wherever you call home. Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers and Doctors provide symptom management, patient and family support while coping with an end-of-life illness. Requirements

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
• Current driver's License.
• Proof of current CPR.
• Must read, write and comprehend English.
• Reliable transportation.
• Valid and current auto liability insurance.

Nice To Haves

• Two years experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
• Nursing skills as defined as generally accepted standards of practice.
• Good interpersonal skills.
• Specialized skills preferred in palliation and end-of-life.

Responsibilities

• Preparing and adjusting patient care plans as needed.
• Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
• Maintaining and updating medical charts, patient files, and health records.
• Administering nursing care for patients of all ages in their place of residence.
• Coordinating care with the interdisciplinary group, patient, family and referring agency.
• Assuming the responsibility for coordination of care.
