About The Position

We’re hiring a Hospice RN Case Manager in Dacula, Duluth, and Buford, Georgia who wants meaningful patient relationships, autonomy in the field, and a schedule that supports life outside of work. You’ll step into a clinical role focused on providing hospice care in patients’ homes throughout northern Gwinnett County. You’ll have the support of an experienced interdisciplinary team while maintaining independence in managing your caseload.

Requirements

• Registered Nurse licensed in Georgia.
• Value compassionate, patient-centered care.
• Enjoy working independently while collaborating with a team.

Nice To Haves

• Hospice experience is preferred.
• Home Health, ICU, CCU, Oncology, and Med Surg experience are welcomed.

Responsibilities

• Provide hospice care in patients' homes.
• Manage caseload with independence.
• Conduct an average of 4 patient visits per day.
