This full-time Hospice RN-Case Manager position involves identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. The role requires a minimum of every 15 days of patient/family contact, utilizing teaching, assessment, and intervention skills to provide comfort care at end of life and maximize quality of life. The Case Manager will educate patients and families, update physicians, and ensure appropriate utilization of services and hospice level of care criteria. Responsibilities include administering medications, leading interdisciplinary care groups (IDG), developing and implementing patient care plans, coordinating services, and arranging for necessary equipment and supplies. Accurate documentation in the electronic health record is essential, recording patient experiences through assessments and narratives. The role involves ongoing assessment of patient response to treatments, making necessary changes to interventions, and communicating with physicians, supervisors, and the IDG. Additionally, the Case Manager will supervise Home Health Aides and LPNs, facilitate discharge planning, and potentially conduct OASIS assessments and establish/update plans of care for Palliative Care patients in Wisconsin. The position also requires utilizing proper body mechanics for patient positioning and transporting, and orienting/mentoring new staff. The incumbent must demonstrate knowledge of age-specific care and principles of growth and development.
