About The Position
At Concho Hearts Hospice, we believe in making every day count — not just for our patients, but for our team too. We are currently seeking a full-time Registered Nurse Case Manager who is passionate about providing compassionate, individualized care to patients and families during their most vulnerable moments.
Requirements
- Current Texas RN license in good standing.
- Minimum 1 year of nursing experience (Hospice, Home Health, or ICU/ER preferred).
- Strong clinical judgment and communication skills.
- Empathy, professionalism, and a heart for hospice care.
- Valid driver’s license and reliable transportation.
Responsibilities
- Manage a caseload of hospice patients, providing skilled nursing visits and coordinating all aspects of care.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Educate patients and families on hospice philosophy, care expectations, and symptom management.
- Act as the primary clinical contact for assigned patients, ensuring comfort, dignity, and quality of life.
- Maintain accurate and timely documentation that meets all regulatory and organizational standards.
- Participate in IDG meetings and collaborate with aides, chaplains, social workers, and medical directors.
