About The Position

Vitality Hospice & Palliative Care is seeking a compassionate Hospice RN Case Manager to join their team. This role involves providing clinical leadership for assigned patients, supported by a strong interdisciplinary team. The position is designed to prevent burnout with manageable caseloads, dedicated support staff, defined territories, and a structured weekday schedule. Leadership is clinician-led and understands the demands of hospice nursing.

Requirements

• Active Pennsylvania RN License
• Hospice or home health experience strongly preferred
• Strong clinical judgment & communication skills
• Valid driver’s license & reliable transportation

Responsibilities

• Conduct comprehensive nursing assessments (with Admissions Nurse support)
• Develop and manage individualized plans of care
• Collaborate with physicians, LPNs, social workers, chaplains & aides
• Educate families on symptom management and disease progression
• Participate in IDT meetings
• Maintain timely, compliant documentation
