This full-time Hospice RN-Case Manager position involves identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. The role requires a minimum of every 15 days of patient/family contact, utilizing teaching, assessment, and intervention skills to provide comfort care at end of life and maximize quality of life. The Case Manager will educate patients and families on disease processes, hospice philosophies, and end-of-life care, while updating physicians as needed. Responsibilities include continuous review of patient caseloads to ensure appropriate utilization of services and adherence to hospice level of care criteria, making referrals to other team members, and administering prescribed medications and treatments. The role also involves leading the interdisciplinary care group (IDG) to develop and implement patient care plans, coordinating services with the team, and arranging for necessary equipment and supplies. Accurate and timely documentation in the electronic health record is essential, recording patient experiences through assessments and narratives. The Case Manager will provide ongoing assessment of patient response to treatments, making necessary adjustments to interventions and care plans, and communicating changes to physicians, supervisors, and the IDG. Supervision of Home Health Aides and LPNs, including supervisory visits and instruction, is also a key responsibility. The role facilitates discharge planning and utilizes community resources for continuity of care. For Wisconsin, responsibilities may include OASIS assessments for Palliative Care patients and establishing/updating care plans, as well as conducting initial and comprehensive hospice assessments. The position requires the use of proper body mechanics and safety techniques for patient handling and may involve orienting and mentoring new staff. The incumbent must demonstrate knowledge and skills for age-appropriate patient care, understanding growth and development principles and assessing patient needs based on age-specific requirements.
