PRN Hospice RN Case Manager

As a Hospice Registered Nurse Case Manager, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, and educating patients and families. Requirements
- Active RN license in the State of Michigan
- 1+ years of hospice nursing experience
- Strong communication, assessment, and organizational skills
- Compassionate, dependable, and mission-minded
- Valid driver’s license and reliable transportation required
