Spirit Hospice Nurse Case Manager balances expert-level compassion with a sense of vibrancy for life. This role is the clinical anchor for patients and families, helping them feel supported, informed, and genuinely cared for every step of the way. The team values teamwork, communication, snacks in the breakroom during IDT, and most importantly heart. Requirements
- Current RN license in the State of Georgia
- Current CPR & First Aid Certification
- Reliable and legal transportation to and from patient's place of residence
Nice To Haves
- Hospice or home care experience
Responsibilities
- Serve as the clinical anchor for patients and families.
- Provide support, information, and care to patients and families throughout their end-of-life journey.
