About The Position

We’re hiring a Hospice RN Case Manager who wants flexibility, meaningful patient care, and long-term stability. You’ll step into a clinical role focused on providing hospice care in patients’ homes throughout Sandy Springs, Dunwoody, Peachtree Corners, Norcross, Doraville, and the surrounding communities. You’ll partner with an experienced interdisciplinary care team while maintaining autonomy in managing your caseload. This role offers a Monday-Friday schedule with an average of 4 patient visits per day within a localized territory to minimize travel. The organization is growing with strong leadership support and established workflows and documentation systems.

Requirements

• Registered Nurse with an active Georgia license.
• Excellent communication skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently while collaborating with a multidisciplinary team.

Nice To Haves

• Hospice experience is preferred.
• Home Health, ICU, CCU, Oncology, and Med Surg experience are welcomed.
