Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum 1 year of experience as a nurse in a clinical care setting
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
  • Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
  • Reliable transportation and valid automobile insurance coverage

Nice To Haves

  • BSN preferred
  • Hospice or home health experience preferred

Responsibilities

  • Administers skilled nursing care to terminally ill patients and their families who meet hospice admission criteria in a home or facility setting.
  • May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice.
  • Responsible for overall coordination of care for their patient caseload.
  • Makes regular visits to patients wherever they reside.
  • Communicates regularly with the Interdisciplinary Team and Hospice Medical Director.
  • Identifies patient, family, and caregiver needs and provides for those needs in accordance with the attending physician's orders and the IDG plan of care.
  • Builds rapport and communicates effectively with patients, families, physicians, facilities, and other IDG team members.
  • Completes initial evaluation and re-evaluates the patient’s nursing needs during each visit.
  • Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
  • Completes assessments specific to the provision of Hospice care, including identification of home safety risks, appropriateness of home environment, availability of support systems, and community resources.
  • Provides appropriate support at the time of death and performs bereavement assessment if requested.
  • Provides positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
