Requirements

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Current State License as a Registered Nurse
Minimum 1 year of experience as a nurse in a clinical care setting
Nursing skills as defined by accepted nursing standards
Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
Reliable transportation and valid automobile insurance coverage

Hospice or home health experience preferred

Responsibilities

Administers skilled nursing care to terminally ill patients and their families who meet hospice admission criteria in a home or facility setting.
May be required to perform admission, routine, respite, or continuous care visits and assess patients within regulatory requirements for hospice.
Responsible for overall coordination of care for their patient caseload.
Makes regular visits to patients wherever they reside.
Communicates regularly with the Interdisciplinary Team and Hospice Medical Director.
Identifies patient, family, and caregiver needs and provides for those needs in accordance with the attending physician's orders and the IDG plan of care.
Builds rapport and communicates effectively with patients, families, physicians, facilities, and other IDG team members.
Completes initial evaluation and re-evaluates the patient’s nursing needs during each visit.
Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
Completes assessments specific to the provision of Hospice care, including identification of home safety risks, appropriateness of home environment, availability of support systems, and community resources.
Provides appropriate support at the time of death and performs bereavement assessment if requested.
Provides positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
