Requirements

Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
Current State License as a Registered Nurse
Minimum one (1) year experience as a nurse in a clinical care setting
Nursing skills as defined by accepted nursing standards
Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
Reliable transportation and valid automobile insurance coverage
Ability to sit, stand, bend, lift, and move intermittently
Ability to lift at least 35lbs and bear the weight of an average adult effectively

Nice To Haves

BSN preferred
Experience as a nurse in a hospice or home health environment is preferred
Familiarity with Home Care Home Base (HCHB)

Responsibilities

Administer skilled nursing care, case management, and support to terminally ill patients and their families in a home or facility setting.
Access, plan, coordinate, implement, and evaluate patient care plans.
Lead an Interdisciplinary Team, including a Hospice Aide, Medical Social Worker, Chaplin, LPN, Music Therapist, Massage Therapist, and Volunteers, in providing appropriate care for the patient, in collaboration with the Hospice Medical Director.
Complete initial evaluations and re-evaluate patient's nursing needs during each visit.
Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
Complete assessments specific to Hospice care, including identification of home safety risks, appropriateness of the home environment, and availability of support systems and community resources.
Provide appropriate support at the time of death and perform bereavement assessment if requested.
Provide positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
