Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum one (1) year experience as a nurse in a clinical care setting
  • Nursing skills as defined by accepted nursing standards
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
  • Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
  • Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines.
  • Reliable transportation and valid automobile insurance coverage
  • Ability to sit, stand, bend, lift, and move intermittently
  • Ability to lift at least 35lbs and bear the weight of an average adult effectively

Nice To Haves

  • BSN preferred
  • Experience as a nurse in a hospice or home health environment is preferred
  • Familiarity with Home Care Home Base (HCHB)

Responsibilities

  • Administer skilled nursing care, case management, and support to terminally ill patients and their families in a home or facility setting.
  • Access, plan, coordinate, implement, and evaluate patient care plans.
  • Lead an Interdisciplinary Team, including a Hospice Aide, Medical Social Worker, Chaplin, LPN, Music Therapist, Massage Therapist, and Volunteers, in providing appropriate care for the patient, in collaboration with the Hospice Medical Director.
  • Complete initial evaluations and re-evaluate patient's nursing needs during each visit.
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the patient plan of care.
  • Complete assessments specific to Hospice care, including identification of home safety risks, appropriateness of the home environment, and availability of support systems and community resources.
  • Provide appropriate support at the time of death and perform bereavement assessment if requested.
  • Provide positive, supportive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
