About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families throughout Licking County. This full-time position offers the opportunity to make a meaningful difference by supporting patients during their end-of-life journey as part of an interdisciplinary care team. This is a field-based role that requires traveling to patient homes and care facilities throughout the Licking service region.

Requirements

• Graduate of an accredited nursing program
• Current Registered Nurse (RN) license in the State of Ohio in good standing
• Current CPR certification
• Basic computer skills and ability to accurately document patient care
• Ability to travel and drive in various weather and road conditions
• Valid driver's license and safe driving record
• State minimum automobile insurance coverage
• Ability to pass a criminal background check
• Ability to pass a 10-panel drug screening

Nice To Haves

• Two years of acute care nursing experience preferred
• Hospice and Palliative Nursing Certification (CHPN) preferred

Responsibilities

• Planning, coordinating, and delivering patient care using the nursing process, including assessment, planning, implementation, intervention, and evaluation.
• Working closely with patients, families, physicians, and other healthcare professionals to ensure compassionate, patient-centered care while maintaining professional nursing standards.
