About The Position

Grace Hospice is looking for a compassionate and adaptable Hospice Resource RN to join their care team. This role is ideal for a nurse who enjoys a dynamic position and serving as a trusted resource for patients, families, and clinical teams. The Hospice Resource RN will be responsible for completing hospice admissions, conducting patient visits as needed, and collaborating closely with Intake, Clinical Managers, RN Case Managers, and the interdisciplinary team to ensure seamless transitions and coordinated care. Requirements

  • Graduate of an accredited school of nursing.
  • Current Minnesota RN license.
  • Three years of recent nursing experience in acute care, long-term care, home care, hospice or public health preferred.
  • Strong assessment, care-planning and patient-education skills.
  • Excellent communication, critical-thinking and problem-solving abilities.
  • Comfortable working independently while collaborating closely with a clinical team.
  • Experience using electronic health records and standard computer applications.
  • Valid Minnesota driver’s license, auto insurance and daily access to reliable transportation.

Nice To Haves

  • Hospice or home care experience is especially valuable, along with a genuine desire to provide compassionate, patient-centered care.

Responsibilities

  • Complete hospice admissions and help patients and families feel informed and supported as they begin hospice care.
  • Provide follow-up and patient visits based on changing care needs.
  • Collaborate with Intake, Clinical Managers, RN Case Managers and the interdisciplinary team to coordinate care and schedules.
  • Respond to patient, family and facility requests for clinical support.
  • Assess and communicate changes in patient condition or needs to the appropriate members of the care team.
  • Obtain physician orders and complete required documentation in the electronic health record.
  • Adapt to changing patient and team needs while helping maintain continuity of care.
