About The Position

AdvisaCare is seeking a passionate Hospice RN Case Manager for their Tucson Hospice team. This role focuses on providing patient-centered end-of-life care, emphasizing comfort, dignity, and peace. The RN will collaborate with patients, families, and the interdisciplinary care team to develop and implement individualized care plans. Key aspects of the role include symptom management, easing transitions, and offering emotional support.

Requirements

Active Registered Nurse (RN) license.
Current and unrestricted Registered Nurse license in the state of Arizona
Minimum of 1 year of experience in hospice or palliative care or 1-2 years of critical care
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work effectively in a team environment
Proficiency in electronic medical record systems
Availability to work evenings, weekends, and rotating shifts as needed

Responsibilities

Provide compassionate education and emotional support to patients and their families throughout their journey.
Facilitate and actively participate in interdisciplinary team meetings to optimize care plans.
Ensure accurate, timely clinical documentation in alignment with company policies and regulatory standards.
