About The Position

Weekend Warrior Hospice Registered Nurse Weekend schedule. Flexible hybrid role. Variety in your day. This is a Weekend Warrior position with a flexible hybrid structure. The role will support on-call operations, with responsibilities that may include a mix of remote and in-field support based on team needs. There is also the opportunity to be cross-trained on back-office functions.

Requirements

  • Maintains the agency's mission, philosophy and core values.
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
  • Always maintains patient confidentiality, including all HIPAA regulations.
  • Licensed or registered in the state in which currently practices
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • Current CPR card; TB testing per agency policy;

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing care in accordance with hospice standards and physician orders
  • Coordinate care with physicians, caregivers, and interdisciplinary team members
  • Educate patients and families on symptom management, disease progression, and end-of-life care
  • Document assessments, interventions, and care plans accurately and timely in the EMR - Homecare Homebase (HCHB)
  • Participate in interdisciplinary team meetings and care planning
  • Support after-hours patient needs as part of an on-call rotation when applicable
