About The Position
Sage Group HealthCare Staffing is seeking an RN Hospice for a 13-week travel/contract position in West Plains, MO. This role involves providing and coordinating care for assigned patients in their homes, managing a caseload, and developing nursing care plans. The position focuses on supporting patient comfort and dignity for terminally ill patients, educating them on disease management, and providing hospice nursing care within established standards. Key responsibilities include assessing patient status, documenting visits within 24 hours, notifying physicians of changes, and offering bereavement support to families. The nurse will cover territories including West Plains, MO, and potentially Mountain Grove, MO.
Requirements
- Registered Nurse: 1 year - Required
- Hospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
- Must have valid Driver’s License and Personal Auto Insurance
- ASN or an equivalent – Required
- RN License for State applying or Compact if accepted in the state - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- Home Care Home Base (HCHB) charting - Highly Preferred
- BSN or an equivalent – Highly preferred
Responsibilities
- Provide and coordinate care for assigned patients in the comfort of their homes.
- Manage assigned patient case load.
- Develop and implement nursing care plans, including Start of Care and admissions, as assigned.
- Support patient comfort and dignity for terminally ill patients.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient’s families as needed.
