About The Position

PRN Hospice Registered Nurse (RN) Location: Greater Phoenix, AZ Position Type: PRN (Per Diem) Make Every Visit Matter. Requirements

Current Arizona Registered Nurse (RN) license.
Minimum one year of RN experience (hospice or home health experience preferred).
Associate's or Bachelor's degree in Nursing.
Current CPR certification.
Valid driver's license, reliable transportation, and current auto insurance.
Strong clinical judgment, excellent communication skills, and a compassionate, patient-centered approach.
Ability to work independently while collaborating with a dedicated hospice team.

Nice To Haves

Hospice or home health experience preferred.

Responsibilities

Provide skilled hospice nursing care in patients' homes and residential settings.
Complete comprehensive nursing assessments and develop individualized plans of care.
Manage pain and symptoms while promoting comfort and dignity.
Educate and support patients, families, and caregivers throughout the hospice journey.
Collaborate with physicians, social workers, chaplains, CNAs, and the interdisciplinary team.
Complete timely, accurate electronic documentation.
Participate in shared on-call coverage as needed.
