Requirements

Current RN license in the state of practice.
Minimum of 1–2 years of nursing experience (hospice or palliative care preferred).
Strong clinical assessment and critical thinking skills.
Exceptional communication skills, with the ability to listen deeply and respond with empathy.
Comfort working independently in varied home and facility environments.
A valid driver’s license and reliable transportation.

Hospice or palliative care experience preferred.

Responsibilities

Provide skilled nursing care to hospice patients in their homes, assisted living facilities, or inpatient settings.
Assess patient conditions, develop individualized care plans, and adjust treatments as needs change.
Administer medications, manage pain and symptoms, and educate families on care techniques.
Collaborate closely with physicians, social workers, chaplains, and other members of the interdisciplinary team.
Offer emotional support and guidance to patients and their loved ones, helping them navigate end-of-life decisions.
Maintain accurate, timely documentation in compliance with regulatory and organizational standards.
