About The Position
Hospice Registered Nurse (RN) - Azura Hospice (Verona Community) Make a Difference in Someone's Final Journey At Azura Hospice, we believe in providing compassionate, dignified end-of-life care that honors each patient's unique story. We're seeking a dedicated Hospice Registered Nurse (RN) to join our compassionate team, delivering exceptional care to patients and meaningful support to their families throughout the Verona Area.
Requirements
- Active RN license in Wisconsin
- Minimum 1-2 years of nursing experience
- Excellent assessment, communication, and time-management abilities.
- Ability to travel locally to patient homes/facilities and participate in on-call rotations as needed.
Nice To Haves
- BSN preferred but not required
- Hospice, palliative care, or home health experience a plus
- A heart for end-of-life care with emotional resilience and empathy.
Responsibilities
- Deliver skilled nursing care, including pain management, symptom control, and emotional support tailored to each patient's needs.
- Work closely with an interdisciplinary team-physicians, social workers, and chaplains-to create personalized care plans.
- Educate and support patients' loved ones with compassion and understanding.
- Maintain accurate, timely records to ensure seamless care coordination.
- Offer presence, empathy, and advocacy for patients in their home or facility settings.
