This role involves providing skilled nursing care to patients receiving end-of-life (EOL) care in facility and home settings. The nurse will respond to phone calls regarding symptom concerns, changes in condition, and emergent situations. Responsibilities include conducting home and facility visits for assessment and management of critical symptoms, providing crisis nursing interventions, conducting death visits, completing after-hours admissions, facilitating hospital discharges, and assisting the RN Case Manager with the plan of care. The nurse will also monitor patient status, respond to facility escalations, provide support for neighboring locations, participate in Interdisciplinary Group (IDG) meetings, reinforce patient and family education, and maintain accurate clinical documentation. Collaboration with patients, families, caregivers, physicians, facilities, and the interdisciplinary team is essential.
