About The Position

University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to provide expert nursing care and support to hospice patients and their families in Westland, Michigan. The role focuses on ensuring comfort and dignity throughout the end-of-life care process. The nurse provides care in patients’ homes and works with an interdisciplinary team and other health professionals to coordinate patient-centered services.

Requirements

• Current and valid Registered Nurse (RN) license in the state of Michigan.
• At least 1 year of nursing experience; hospice, palliative care, or home health experience is preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and make sound clinical decisions.
• Compassionate and patient-centered approach to care.
• Reliable transportation and a valid driver’s license for home visits.

Responsibilities

• Provide skilled nursing care and assessment to hospice patients in their homes.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and family members on care techniques, symptom management, and disease progression.
• Document nursing care and patient observations accurately and thoroughly.
• Participate in regular team meetings and professional development sessions.
• Advocate for patient needs and coordinate care with other health professionals and hospice staff.
