About The Position

As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice, you will play a key role in an organization with a reputation for unsurpassed quality care, a close-knit, family-friendly culture, and a supportive management team as we begin to introduce hospice services to our Toledo, OH territory. We will rely on you to assess, plan and implement patient care; educate patients and families on comfort care processes, procedures and medications; and address various patient needs. You will have the opportunity to build satisfying one-on-one relationships with patients and families by providing comfort care at the end of life. Ohioans Home Healthcare & Hospice is a Medicare and Medicaid Certified and CHAP-accredited skilled home health care agency serving 34 counties in Ohio and Southeast Michigan. Requirements

• One year of comfort care experience
• Current RN state licensure
• CPR certification
• Ability to travel for in-home patient visits
• Exceptional time-management skills
• Excellent interpersonal and customer service skills

Nice To Haves

• Hospice/case management experience preferred
