About The Position

As a home care registered nurse, you provide comfort-focused in-home nursing care for hospice patients with prognoses of six months or less. You help patients remain comfortable during their end-of-life journeys and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians and advocate for patients and families, ensuring their goals of care are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers.

Most of your work takes place wherever patients call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to meet with the interdisciplinary team. Requirements

• Associate or bachelor’s degree in nursing.
• Valid RN license.
• At least one year of hospital nursing experience.
• Reliable transportation.

Responsibilities

• Provide comfort-focused in-home nursing care for hospice patients with prognoses of six months or less.
• Work with an interdisciplinary hospice team, including LPNs, aides, chaplains, social workers, and physicians.
• Advocate for patients and families and ensure their goals of care are communicated and prioritized by the care team.
• Provide basic hospice education and training to patients, families, and caregivers.
• Make typically 4–6 patient visits each day and attend a weekly meeting at the local program office.
