As a home care registered nurse, you provide comfort-focused nursing care to hospice patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians. You advocate for patients and families, ensuring their goals of care are communicated and prioritized, and provide basic hospice education and training to patients, families, and caregivers. Most care takes place wherever patients call home, including private residences and assisted living or skilled nursing facilities. The role typically involves 4–6 visits each day and a weekly visit to the local program office to meet with the interdisciplinary team, which collaborates to provide holistic support to patients and families.
