About The Position

AdvisaCare is seeking a full-time Hospice Registered Nurse to serve patients in Lansing and surrounding areas, with the Troy office as the parent agency. The RN provides individualized hospice care in patients’ familiar surroundings, supporting their comfort and dignity and building close relationships with patients and families. The role includes assessing patient needs, implementing and revising care plans, educating and supporting patients and families, and coordinating care with the hospice team. The nurse also conducts supervisory visits, contributes to interdisciplinary group meetings, and provides clinical oversight of assigned team members.

Requirements

Current Michigan registered nurse license in good standing.
At least 1 year of hospice experience.
Empathetic approach to patients and caregivers.
Ability to adapt to patients’ changing status and needs while providing full-time on-call coverage.

Responsibilities

Conduct initial and ongoing assessments of patients’ physical, functional, psychosocial, and environmental needs.
Implement individualized plans of care and make revisions as needed.
Consult with and educate patients and families about disease processes, self-care techniques, end-of-life care, and ethical concerns.
Supervise paraprofessionals providing services to patients.
Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences.
Conduct appropriate supervisory visits, including clinical observation of assigned team members’ performance.
Participate in hospice performance improvement activities.
Maintain a high level of customer satisfaction with clients, families, and caregivers.
Communicate information using the organization’s current processes and technology.
Participate in daily field visits and weekly office meetings as indicated.
Provide emotional support to help patients live their final days with dignity and comfort.
