As a home care registered nurse, you provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You help patients remain comfortable during their end-of-life journeys and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians, and advocate for patients and families so their goals of care are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers. Most of your time is spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to meet with the interdisciplinary team, which collaborates to provide holistic support for patients and families.
