About The Position

AHN Healthcare@Home is hiring a Registered Nurse to provide end-of-life care to patients in their homes in the Ross Township, Pennsylvania area. The nurse delivers patient-centered hospice care focused on comfort, dignity, and quality of life, collaborating with an interdisciplinary team while working independently in the field. The position includes a $10,000 first-year success bonus for achieving key milestones. Field education and leadership support are provided from the start, with opportunities for professional growth and input into workplace improvements.

Requirements

Active Registered Nurse (RN) license in the state where the position is performed.
Two years of RN experience and clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.

Hospice experience preferred.

Responsibilities

Deliver compassionate, patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
Educate and support patients and families throughout the end-of-life journey.
Think critically and solve problems in real time.
Maintain thorough, compliant EMR documentation.
Travel to patient homes within the assigned territory.
