About The Position

HopeWest is seeking a Hospice Registered Nurse for their Grand Junction Community Hospice team. This role focuses on relationship-based care, teamwork, and providing compassionate care to patients and families. The Hospice Nurse acts as a teacher, helping patients identify goals of care and empowering caregivers. They are specialists in symptom management, requiring strong assessment and critical thinking skills. Primary RNs manage an assigned caseload of patients and lead the interdisciplinary team, providing care in various settings including private homes, nursing homes, group homes, and assisted living facilities. Nurses in this role appreciate the Monday-Friday schedule, lack of traditional hospital shift rotations, relationship-based care model, collaborative team, meaningful work, mission-driven culture, supportive leadership, and the autonomy to plan their day.

Requirements

  • Minimum 1–2 years of nursing experience preferred
  • Experience in home health, hospice, or acute care strongly preferred
  • Current Colorado RN license maintained throughout employment
  • Current CPR certification maintained throughout employment
  • Valid Colorado driver’s license, reliable transportation, and proof of insurance
  • Strong communication and interpersonal skills
  • Ability to navigate emotional or high-stress situations with professionalism and compassion
  • Experience with EHR systems and Microsoft Office applications

  • Provide compassionate care to hospice patients and their families.
