About The Position

Halcyon Home is seeking a dedicated and compassionate Hospice Registered Nurse Full Time to support our growing geriatric hospice population in Dallas, TX. This role involves providing comprehensive care, implementing care plans, and educating patients and families about end-of-life care. The ideal candidate is a Registered Nurse with a passion for seniors, strong communication skills, and experience in home health or a related field. Requirements

  • Current RN license and good standing with the state nursing board

Nice To Haves

  • Knowledge or experience utilizing Homecare Homebase EMR would be a plus
  • Experience in home health or related healthcare setting preferred

Responsibilities

  • Perform initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs related to their terminal disease.
  • Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
  • Implement, develop, and document the plan of care, ensuring quality and continuity of care, and recommend revisions as necessary.
  • Provide effective communication to patients, families, team members, and other healthcare professionals.
  • Apply concepts of infection control and standard precautions in coordinating and performing patient care activities.
  • Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service.
  • Prepare clinical and progress notes demonstrating progress toward established goals and ensure continuity of quality patient care with appropriate documentation.
  • Inform the physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.
  • Monitor assigned cases to ensure compliance with third-party payor requirements.
  • Demonstrate commitment, professional growth, and competency.
  • Promote Agency philosophy and administrative policies.
  • Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Determine the scope and frequency of services needed based on acuity and patient/family needs.
  • Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
