Requirements
- Active and unrestricted New York State Registered Nurse (RN) license
- Minimum 2 years of recent experience in one or more of the following: Hospice, ICU, Med-Surg, Community Health, Oncology
- Strong clinical assessment, critical thinking, and care planning skills
- EMR proficiency (WellSky experience preferred, but training will be provided)
- Recent physical exam (within 1 year)
- Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years)
- MMR immunization documentation
- Valid driver’s license and reliable transportation
- Authorized to work in the United States
- Ability to work independently in a field-based setting
- Hands-on experience with IV therapy
- Hands-on experience with Midline catheter management
- Hands-on experience with Tracheostomy care
- Hands-on experience with Foley catheter care
- Hands-on experience with Ostomy care
- Hands-on experience with Nephrostomy tube care
- Hands-on experience with Tenckhoff catheter management
- Hands-on experience with Tube feeding administration
- Hands-on experience with Chest tube management
- Hands-on experience with Advanced wound care
- Hands-on experience with Pain and symptom management

Responsibilities
- Provide skilled hospice nursing care to patients in their homes
- Conduct follow-up hospice visits
- Administer medications and treatments as ordered
- Provide wound care management
- Perform IV therapy and related interventions
- Conduct patient pronouncements when appropriate
- Monitor and manage pain and symptom control
- Collaborate with interdisciplinary hospice team members
- Educate patients and families on care plans and symptom management
- Maintain accurate and timely clinical documentation
- Provide emotional support to clients and families
