As a home care registered nurse, you provide comfort-focused nursing care to hospice patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You work with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians. You advocate for patients and families by ensuring their goals of care are communicated and prioritized, and provide basic hospice education and training to patients, families, and caregivers. Most care takes place wherever patients call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to collaborate with the interdisciplinary team in providing holistic support to patients and families. VITAS Healthcare describes its commitment as delivering compassionate, personalized care with heartfelt empathy and honoring each patient's wishes. The organization has provided end-of-life care since 1978 and operates programs in more than 15 states. It promotes from within and describes its employee experience as award-winning and nationally recognized.
