As a home care registered nurse, you will provide comfort-focused in-home nursing care to hospice patients with prognoses of six months or less. You will help patients feel comfortable during end-of-life care and build compassionate, meaningful connections with patients and the people in their lives. You will advocate for patients and families, ensure their care goals are communicated and prioritized, and provide basic hospice education and training to patients, families, and caregivers. You will work with an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians. Most of your time will be spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You will typically make 4–6 visits each day and visit the local program office once a week to meet with the interdisciplinary team and collaborate on holistic support for patients and families.
