About The Position

AdvisaCare is expanding its services to include Hospice care and is seeking compassionate, dynamic, energetic, organized, and efficient professionals with Hospice experience to join their team. This role involves providing emotional support to patients in their final days, ensuring dignity and comfort. The Hospice RN will conduct supervisory visits, provide feedback in interdisciplinary group meetings, and contribute to the development and revision of patient/family care plans. The agency is looking for individuals who understand the value of Hospice care and are eager to make a difference.

Requirements

Current Michigan License in good standing
Minimum 1 year hospice experience
Empathetic approach to patients and caregivers
Ability to adapt to patient’s status and needs on a full time on call basis

Responsibilities

Conduct initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs.
Implement the individualized plan of care and revisions as needed.
Ensure compliance with AdvisaCare Hospice Policies and Procedures, and Medicare compliance.
Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and dealing with issues of ethical concern.
Supervise paraprofessionals providing services to patients.
Prepare clinical and progress notes.
Participate in IDG (Interdisciplinary Group) conferences.
Participate in the Hospice performance improvement program.
Maintain a high level of customer satisfaction with clients, families, and caregivers.
Communicate information using current processes and technology within the organization.
Conduct appropriate supervisory visits for clinical observation of performance of assigned team members.
Participate and provide feedback to the team for regular interdisciplinary group meetings on the development, review, and revision of the patient/family plan of care.
