About The Position

Suncrest Hospice is seeking a Registered Nurse to provide exceptional clinical care to its patients. This role focuses on delivering care and services to a group of patients in their place of residence, which may include private homes, skilled nursing facilities, and assisted living facilities. The Registered Nurse Case Manager is a vital part of the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources. Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance.
  • Ability to travel across the service area.

Responsibilities

  • Provide care and services for a group of patients.
  • Visit patients in their place of residence (private homes, Skilled Nursing Facilities, assisted living facilities).
  • Serve as a contributing member of the interdisciplinary team.
  • Provide expertise regarding psych-social assessments and interventions.
  • Provide knowledge of community resources.
  • Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Work closely with patients on a 1-1 level.
  • Manage visits, documentation, and meetings.
  • Schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns.
  • Communicate via phone, email, and written correspondence.
  • Meet with a variety of healthcare professionals.
  • Understand hospice rules and regulations.
