About The Position

Interim HealthCare Hospice is hiring a Hospice Registered Nurse to provide home-based care to patients with six months or less to live, with a focus on comfort and quality of life. Requirements

Graduate of an accredited nursing program.
Active RN license in Wisconsin.
At least one year of nursing experience; hospice or home healthcare experience is preferred in the posting.
CPR certification.
Knowledge of state and federal home health regulations.
Clinical proficiency in nursing specialty.
Strong communication skills and compassion.

Responsibilities

Provide home-based care to patients with six months or less to live, focused on quality of life.
Work with an interdisciplinary team that includes a physician, LPN, CNA, HHA, social worker, chaplain, and volunteers to carry out the patient’s plan of care.
Perform patient assessments; check vital signs; evaluate breathing; monitor pain; and administer prescribed medications and treatments.
Ensure that medical equipment, prescribed oxygen, medications, and other supplies are accessible.
Educate families about the patient’s prognosis, changes in the care plan, medication, and hospice goals.
Provide holistic care that engages the patient’s mind, body, spirit, and family.
