Requirements

  • Graduate of an accredited Registered Nurse (RN) program
  • Active South Carolina RN license or Compact License
  • Minimum one year of nursing experience

Nice To Haves

  • Hospice experience preferred

Responsibilities

  • Perform nursing assessments during scheduled, as-needed, or on-call visits to evaluate the patient’s physical condition, symptoms, and immediate care needs.
  • Assess changes in the patient’s condition and promptly communicates significant findings to the RN Case Manager, physician, and/or appropriate members of the interdisciplinary group (IDG).
  • Provide nursing care in accordance with the established plan of care and assists with implementing updates or interventions based on changes in the patient’s condition.
  • Administer medications and treatments as prescribed by the physician and in accordance with hospice policies and nursing standards of practice.
  • Provide interventions to support effective pain and symptom management and evaluates the patient’s response to treatment.
  • Educate patients, families, and caregivers regarding medications, symptom management, comfort measures, disease progression, and when to contact the hospice team for additional support.
  • Coordinate with the RN Case Manager and other IDG members to ensure continuity of care and appropriate follow-up after PRN visits.
  • Respond to changes in patient condition, symptom-management needs, and other urgent clinical concerns within the scope of the PRN assignment.
  • Complete timely and accurate documentation of assessments, interventions, patient/family education, changes in condition, and communication with the hospice care team.
  • Provide compassionate, patient-centered end-of-life care that promotes comfort, dignity, and support for patients and their families.
