About The Position
JourneyCare Hospice is seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care. They offer a different approach with manageable caseloads, strong interdisciplinary support, and dedicated time for patient and family care. This role offers a consistent Monday-Friday schedule with only one weekend rotation every 12 weeks, focusing on quality of care over visit volume, and supportive leadership.
Requirements
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Strong clinical judgment and communication skills
- Valid driver’s license and proof of insurance
Nice To Haves
- Hospice or home health experience preferred
Responsibilities
- Manage a dedicated hospice caseload as the primary RN Case Manager
- Perform patient assessments, admissions, and individualized care planning
- Deliver skilled nursing care focused on comfort and dignity
- Coordinate care with physicians and interdisciplinary team members
- Conduct routine visits and adjust care plans as needed
- Provide end-of-life support, including family education and pronouncement
- Maintain timely, accurate documentation
- Supervise Home Health Aides in compliance with regulations
