About The Position
Companion Hospice is seeking a compassionate and experienced RN Case Manager to guide patients and families through end-of-life care with dignity, comfort, and support. This role offers clinical expertise, autonomy, and the opportunity to make a meaningful impact. Nurses will build deep relationships as the primary point of contact, work in a supportive Interdisciplinary Team (IDG) environment, and enjoy strong autonomy with excellent clinical and leadership support.
Requirements
- Current Oklahoma Registered Nurse license
- At least 1 year of recent medical/surgical, oncology, home health, or hospice experience
- Strong knowledge of pain and symptom management in terminally ill patients
- Excellent communication, organization, and problem-solving skills
- Valid driver’s license, reliable transportation, and current auto insurance
Responsibilities
- Develop and manage individualized Plans of Care with patients, families, physicians, and the IDG
- Coordinate all hospice services and ensure seamless care across settings
- Provide expert pain and symptom management
- Educate patients and families on the Medicare Hospice Benefit and hands-on care
- Supervise Home Health Aides and deliver high-quality care
- Complete timely documentation and participate in admissions
