Empyrean Hospice hires Registered Nurse Case Managers to plan, organize, and direct hospice care with emphasis on community health education and experience. Salary or hourly rate listed is the base for this role; increase commensurate with experience. The Registered Nurse Case Manager will represent Empyrean Hospice with the utmost professionalism and compassion. They will complete an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es). The role involves initiating and developing an individual care plan that establishes goals, including the patient and the family in the planning process. The Case Manager will assume responsibility to coordinate patient care for their assigned caseload, counsel the patient and family in meeting nursing and related needs, and provide health care instructions. They will assist the patient with activities of daily living and facilitate self-sufficiency and optional comfort care. Accurate and relevant clinical notes regarding patient’s condition and care given must be completed, maintained, and submitted. Communication with the physician and hospice care team regarding patient needs and changes in condition is crucial, as is obtaining/receiving physician orders and providing continuity of care. The role also involves providing and maintaining a safe environment for the patient, arranging equipment and services, supervising ancillary personnel, and delegating responsibilities when required. Prioritizing standards of ethical business and clinical practice is essential.
