We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Team in the Media/West Chester, PA, and Surrounding areas. The ideal candidate will have a strong clinical background and a passion for providing exceptional end-of-life care. Our team is committed to providing personalized support and comfort in the familiar surroundings of patients' homes.
Requirements
- An active RN license in the state in which practicing
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Compassionate and empathetic approach to patient care
- Ability to work independently and as part of a team
- Knowledge of hospice regulations and standards of care
- Reliable transportation and willingness to travel
Nice To Haves
- Minimum of 1 year of experience preferred
Responsibilities
- Conducting Start of Care comprehensive assessments
- Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
- Preparing accurate, orderly, and timely documentation
- Coordinating care with patients and provide proper reports to Case Manager
- Provides health care instructions to the patient/family as appropriate per assessment and plan
