About The Position

This role involves providing direct patient care in a hospice setting, focusing on comfort, dignity, and quality of life for patients. The nurse will assess patient needs, manage symptoms, and adjust care plans as necessary. A key aspect of the role is coordinating with a multidisciplinary team, including physicians, hospice aides, LPNs, social workers, and chaplains, as well as providing support and education to patient families. The position requires efficient documentation in the EMR system and allows for autonomy in daily scheduling.

Requirements

  • Active RN license in the state of practice.
  • Graduate of an accredited nursing program (ADN or BSN).
  • 2+ years of RN experience in the hospice setting.
  • Strong clinical judgment, compassion, and communication skills.
  • Reliable transportation.
  • Valid driver's license.
  • Auto insurance.
  • CPR certification.

Nice To Haves

  • Experience working for a hospice agency/program.

Responsibilities

  • Serve as the primary nurse coordinating care for assigned patients.
  • Develop, implement, and update individualized plans of care.
  • Determine visit frequency based on patient acuity and family needs.
  • Provide guidance to Hospice Aides and LPNs to ensure excellent care.
  • Communicate changes in condition to physicians and the care team.
  • Participate in on-call rotation.
  • Visit patients in their homes, focusing on comfort, dignity, and quality of life.
  • Assess patient needs, manage symptoms, and adjust plans of care as conditions change.
  • Coordinate closely with physicians, hospice aides, LPNs, social workers, and chaplains.
  • Support and educate families, helping them feel confident and cared for.
  • Document care clearly and efficiently in the EMR, Consolo.
  • Advocate for patients while maintaining autonomy in how the day is structured.
