About The Position

Sage Group HealthCare Staffing is seeking a Hospice RN CM for a 13-week travel/contract position in Tullahoma, TN. This role involves providing and coordinating care for assigned hospice patients in their homes, developing and implementing nursing care plans, and offering case management services. The goal is to support pain management, enable patients to remain at home, and empower them and their families to reach healthcare goals through education on disease management. The role also includes providing clinical oversight to interdisciplinary teams and ensuring all nursing care and documentation adheres to professional standards and facility policies.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Hospice nursing in a Home Health setting: 1 year within the last 3 years - Required
  • Home Care Home Base (HCHB) charting, hands-on experience and proficiency: 1 year - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • Must be Covid Vaccinated or provide medical or religious exemption
  • ASN or an equivalent – Required
  • RN License for State of Indiana or Compact - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned hospice patients in comfort of their homes.
  • Develop and implement nursing care plans including Start of Care and admissions as assigned.
  • Provide case management services for the assigned patient case load.
  • Support pain management and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
  • Empower patients and families to reach healthcare goals by educating them on disease management
  • Case management and provide clinical oversight to interdisciplinary teams
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status
  • Document and complete charting for all patient visits within 24 hours.
